We have three different tiers for adults, children, and those requiring deeper periodontal treatments/cleanings. Program Exclusions and Limitations

This one-year membership plan includes the services listed on the front and allows 10% off of other treatments not listed in the inclusions, EXCEPT for Invisalign, Tooth Whitening Products/Services, WaterPiks, and Medicaments. Any services not used within the 1-year plan membership are not refundable, not transferrable to another patient, and will not roll over to another plan membership. It is the responsibility of the patient to attend all scheduled appointments in a timely manner in order to utilize the full benefits of the savings plan.
